Summary

This is about changing your name

Body

To change your official name
You may change your name in person at the Registrar's Office in Jardine Hall Room 102. You will need your updated driver’s license with your new name, along with other legals documents supporting the connection between the old and new names (marriage license, divorce decree, court order, etc.).

If you do not have access to come to campus, you can mail notarized copies of the following documents to the Registrar’s Office:

  • Updated driver's license or U.S. Passport
  • The legal document that changed your name (marriage license, divorce decree, etc.)

To change your diploma name
Currently Enrolled Students: The name on your diploma will be the name you submitted in your online Application for Degree (AFD). If you want to change that name, contact the Registrar’s Office. Any name you submit for your diploma name must be a current or past name we have on your academic record.

Former Students: If you already have a diploma with a former name and wish to obtain a new diploma reflecting your name change, please complete the Duplicate Diploma Form. Your name must be officially changed with the Registrar’s Office prior to submitting this form (instructions above).
